REQUIREMENTS AND FORMS  FOR CONFIRMATION

The forms mentioned below will be given to the Candidates at their January Class:

Baptismal Information Form

Confirmation Candidate Information Form

Confirmation Sponsor Form

Confirmation Essay

Proof of Service Hours for Year I and Year II  (On Green Service Hour Form) 10 Hours each year. 

(Year I service hour forms should be handed in by the November Class.)

Proof of Service Hours at Preakness Healthcare Center (On Green Service Hour Form) 10 hours – Year I – Should be handed in by the November class.

Proof of Service Hours at New Jersey Food Bank – 2 hours – On Green Service Hour Form

Make up work for any session that has been missed.

Confirmation Retreat attendance – 1 Retreat is required.

All of the above must be handed into to their Confirmation Catechist by March 15, 2009.  The Catechist will then bring it to the Confirmation Office where it will be checked in.    The candidate will then be scheduled for a final Confirmation Interview.

Fall of Confirmation Year III -  2009

September   Sponsor – Candidate Mass and Dinner 

October Confirmation Rehearsal – Mandatory Attendance

October Confirmation –  Date to be set by Bishop
